改良FOLFOX方案新辅助化疗对进展期胃癌手术疗效的影响

目的:探讨奥沙利铂联合氟尿嘧啶、亚叶酸钙方案对进展期胃癌手术疗效的影响。方法:将60例进展期胃癌患者随机分为两组,其中观察组30例,对照组30例。观察组给予新辅助化疗和手术治疗,对照组仅给予手术治疗。观察新辅助化疗的疗效和不良反应,以及其对手术切除率和手术并发症发生率等的影响;并与对照组进行对比。结果:观察组化疗的有效率为76.7%,疾病控制率为96.7%。化疗的主要不良反应骨髓抑制、胃肠道反应、乏力及神经系统毒性,且多为轻度。观察组根治性切除率为83.33%,明显高于对照组的46.67%(P<0.05)。两组手术并发症发生率均较低,且差异无统计学意义(P>0.05)。结论:奥沙利铂联合氟尿嘧啶、亚叶酸钙的新辅助化疗方案对进展期胃癌疗效较好,不良反应可耐受;该方案能提高进展期胃癌的根治性切除率,而未增加手术并发症的发生率。

The influence of neoadjuvant chemotherapy on the surgery effect for patients with gastric cancer

Objective To investigate the influence of neoadjuwant chemotherapy of oxaliplatin,canlcium folinate and flurouracil on the surgery effect for patients with gastric cancer.Methods 60 patients with gastric cancer were divided into two groups.The observation group:30 patients received the neoadjuvant chemotherapy and operation;the control group:30 patient received the operation.The effect and toxicities neoadjuvant were evaluated.The radical resection rate,complications incidence rate were conpared between the two group.Results In the observation group,the response rate and disease control rate of neoadjuvant chemotherapy was 76.7% and 96.7%,respectively.The main chemotherapy toxicities were marrow depression,gastrointestinal reaction,anergy and neurotoxicity,they were tolerated.The radical resection rate of the observation group(83.33%) was than that of the control group(46.67%)(P<0.05).There was no significant chang in the complications incidence rate between the two groups(P>0.05).Conclusion Oxaliplatin,calcium folinate and flurouracil neoadjuvant chemotherapy is effective and safe for gastric cancer;it can improve the radical resection rate of gastric cancer,and do not improve the conplications incidence rate.
