社区糖尿病管理模式效果分析

目的:探讨糖尿病患者社区管理的治疗效果。方法:抽取社区180例2型糖尿病患者为社区管理组,按照同等病情抽取同期中心慢病门诊100例2型糖尿病患者为中心门诊组。比较两组管理和治疗1年后的病情控制及自我管理行为情况。结果:观察组患者1年后空腹血糖水平较管理前及同期对照组血糖水平均明显降低,差异有统计学意义(P<0.05),血糖控制达标率显著高于对照组(61.1%vs.42.0%,P<0.01)。社区糖尿病管理后观察组自我管理行为各维度评分较管理前均显著改善,差异有统计学意义(P<0.05或0.01),除遵嘱用药外各维度评分均较对照组明显提高(P<0.05或0.01)。结论:社区糖尿病管理可有效改善患者自我管理行为,提高血糖控制率,值得进一步完善应用。

Effectiveness analysis of diabetes managing model in community

Objective To explore the therapeutic effect of diabetes for community management. Method 180 patients with type II dia- betes were collected as community group. Meanwhile, 86 patients from hospitals with the similar condition were selected as hospital group. The disease control and serf management behavior of the two groups were compared after I year＇s management and treatment. Results The level of serum fasting serum glucose was significantly lower in observation group than that in control groupand beffore coummnity man- aging（P〈 0.05）. And the standard blood sugar control rate of observation was significant higher than that in control group （61.1% vs. 42. 0% ,The diffetence was statistically significantP 〈 0. 01 ）. Compared with before managment, scores on all sectors of self - manage- ment behavior of observation group were significant inpmved （P 〈0. 05 or 0. 01 ） ,and except take medicine each demension score were sig- nificantly increased than that in control group （P 〈0. 05 or 0. 01 ）. Conclusion Diabetes patients with community management can effec- tively improve self management behavior and blood sugar control,it＇s worth further application.
