新生儿黄疸血清总胆汁酸测定的状况分析

摘    要:目的:分析血清总胆汁酸的测定在新生儿黄疸实验室诊断中的临床意义。方法:将98例黄疸患儿分为两组,其中生理性黄疸组19例,病理性黄疸组79例,对照组36例,测定血清总胆汁酸的水平并分析比较。结果:36例对照组黄疸患儿血清总胆汁酸(TBA)结果为(7.23±2.98)μmol/L,79例病理性黄疸组TBA结果为(36.75±29.56)μmol/L,19例生理性黄疸组TBA结果为(7.48±7.25)μmol/L,病理性黄疸患儿的血清总胆汁酸水平明显高于对照组,差异有统计学意义(P<0.01);对照组TBA与生理性黄疸组比较差异无统计学意义(P>0.05)。结论:在新生儿黄疸实验室诊断中血清总胆汁酸测定具有较好的特异性,为临床提供诊断和治疗依据,对于减少高胆红素血症患儿并发症的发生,具有一定的临床价值。

Abstract:Objective To analyze the clinical significance of serum total bile acid determination in the laboratory diagnosis of jaundice in the newborns.Method For 98 cases of jaundice(19 cases of physiological jaundice,79 cases of pathologic jaundice),and 36 cases in the control group,were detected the serum total bile acid levels and compared.Results The serum total bile acid(TBA) levels were(7.23 ±2.98) μmol/L,(36.75±29.56) μmol/L and(7.48±7.25) μmol/L,respectively in the control group,TBA pathologic jaundice group and TBA physiological jaundice group.The serum TBA level in TBA pathologic jaundice group was obviously higher than the control group(P<0.01);the difference between the control group and TBA physiological jaundice group was not statistically significant(P>0.05).Conclusion In the laboratory diagnosis of neonatal jaundice,determination of serum total bile acid has highly specificity,provides evidence for clinical diagnosis and treatment,which shows some clinical value in reducing the complication incidence of children with hyperbilirubinemia.
