摘    要:目的 探讨应用PDCA循环管理法促进我院新生儿科抗菌药物的合理使用.方法 应用PDCA循环管理的方法通过Plan(计划)、Do(执行)、Check(检查)和Action(处理)4个阶段规范新生儿科抗菌药的使用,观察并比较实施前后相关指标的变化.结果 新生儿科抗菌药物使用率下降12.3%,抗菌药物使用强度下降24%,抗菌药费用占总药费比例下降26.9%,特殊抗菌药的使用率下降49.4%,使用特殊级抗菌药送检率增加53.2%,且抗菌药不合理使用率明显降低(P<0.05).结论 PDCA循环管理能有效促进新生儿科抗菌药物的合理应用,符合医疗质量持续改进的要求,值得在抗菌药物临床管理中推广.

Abstract:Objective To explore the effect of Plan-Do-Check-Action ( PDCA) cycle on the management of antibiotics usage in neonatal department. Methods PDCA mode was used to guide the rational usage of antibiotics in neonatal department, and the changes of some relevant indicators were analyzed before and after application of PDCA management. Results After the application of PDCA method, the antibiotics utilization rate and intensity were decreased by 12. 3% and 24%, respectively. The ratio of antibiotics on total drug cost fell by 26.9%, and the special antibiotics ratio fell by 49.4%. The bacteria examination rate of using special antibiotics was increased by 53. 2%. The proportion of related unreasonable parameters decreased obviously ( P<0.05) . Conclusion PDCA cycle management can effectively promote the rational application of antibiotics in neonatal department, which accords with the requirement of medical quality continuous improvement. The PDCA cycle management is worth popularizing in the clinical management of antibiotics application.
