髂内动脉结扎治疗骨盆骨折大出血合并腹腔脏器损伤效果分析

目的:探讨髂内动脉结扎治疗骨盆骨折大出血合并腹腔脏器损伤效果,为后续临床治疗提供参考.方法:11例骨盆骨折大出血合并腹腔脏器损伤患者于2017年1月-2018年1月期间在我科室接受治疗,所有患者均采取髂内动脉结扎治疗,分析临床效果.结果:1例患者死亡,6例出血控制,2例出血减少,2例无效,总有效率为72. 7% ,术后开展12个月随访,出现神经麻痹足下垂1例,出现跛行1例,其余8例患者骨盆外形以及下肢功能良好患者,临床治疗满意率为90. 0% .结论:骨盆骨折大出血合并腹腔脏器损伤患者在急救或者复苏剖腹术中及时进行髂内动脉结扎治疗可控制出血量,提高生存率.

Analysis of effect of ligation of internal iliac artery on pelvic fracture and massive hemorrhage combined with abdominal organ injury

Objective:To investigate the effect of internal iliac artery ligation in the treatment of pelvic fracture and massive hemorrhage combined with abdominal organ injury,so as to provide reference for subsequent clinical treatment.Methods:11 patients with pelvic fracture and massive hemorrhage combined with abdominal organ injury received treatment in our department from January 2017 to January 2018.All patients received ligation of the internal iliac artery to analyze the clinical effect.Results:1 patient died,6 patients had bleeding control,2 patients had bleeding reduction,2 patients had no effect,and the total effective rate was 72.7%.After 12 months of follow-up,1 patient had neuropalsy foot drop,1 patient had cllameness,and the rest 8 patients had good pelvic shape and lower limb function,and the clinical treatment satisfaction rate was 90.0%.Conclusion:timely ligation of the internal iliac artery during emergency or resuscitation laparotomy could control the amount of bleeding and improve the survival rate in patients with pelvic fracture and massive bleeding complicated with abdominal organ injury.
