老年人多重耐药鲍曼不动杆菌肺炎的危险因素分析

目的分析老年人多重耐药鲍曼不动杆菌(MDR-AB)肺炎与普通菌感染肺炎危险因素的差异,提高对其防治水平。方法回顾性分析87例老年肺炎病例,根据是否发生MDR-AB感染分成MDR-AB组42例、非MDRAB组45例。对可能影响MDR-AB感染发生的18项相关因素先行单因素分析,比较相关因素在MDR-AB组和非MDR-AB组间有无统计学意义。然后将单因素分析差异有统计学意义的相关因素行非条件Logistic多因素回归分析,校正混杂因素影响。结果单因素分析显示:脑血管病及痴呆病史、低蛋白血症、吞咽困难、长期卧床、近期ICU入住史、近期应用3/4代头孢菌素、有创操作等7项因素在MDR-AB组与非MDR-AB组之间差异有统计学意义(P<0.05)。进一步行非条件Logistic多因素回归分析后显示,低蛋白血症(OR=5.083,95%CI 1.223²1.123)、长期卧床(OR=5.692,95%C11.13221.123)、长期卧床(OR=5.692,95%C11.132²8.617)为独立危险因素。结论低蛋白血症、长期卧床为老年人MDR-AB肺炎的独立危险因素。

Analysis of the risk factors in multiple drug resistant Bowman acinetobacter pneumonia in the elderly patients

Objective To analyze elderly acinetobacter baumannii multidrug-resistant （ MDR-AB） pneumonia and bacterial infections common risk factors for pneumonia differences and improve their prevention and treatment .Methods Ret-rospective analysis of 87 cases of pneumonia in elderly patients , depending on whether MDR AB infection , they were divided into MDR-AB group （42 cases）, non-MDR-AB group of 45 cases.For the 18 factors that could affect the MDR-AB infec-tions, first performed univariate analysis , comparing the relevant factors in the presence or absence of statistical significance between MDR-AB group and non-MDR AB group.Then the univariate analysis was significantly related factors were per-formed with non-conditional Logistic regression analysis , corrected confounding factors .Results Univariate analysis showed that：a history of cerebrovascular disease and dementia , hypoalbuminemia , difficulty swallowing , prolonged bed rest , the re-cent history of ICU stay , the recent application of 3 /4 generation cephalosporins , and other invasive procedures among MDR-AB group and non-MDR-AB group was statistically significant different （ P ＆lt;0.05）.After further performed with non-condi-tional Logistic regression analysis, it showed that hypoalbuminemia （OR=5.083,95% CI 1.223-21.123）, long term bed rest （OR =5.692, 95%CI 1.132-28.617） are independent risk factor.Conclusion Hypoalbuminemia, bedridden are independent risk factor for elderly MDR AB pneumonia .
