经纤维支气管镜局部化疗治疗晚期肺癌

目的　观察经纤维支气管镜 (纤支镜 )局部化疗治疗晚期气道阻塞性肺癌患者的疗效。方法　将 6 0例晚期肺癌患者 ,随机分为观察组 ( 30例 )和对照组 ( 30例 )。观察组采用日本OlympusBF 2 0及配套的NM 3K型注射针在肿瘤局部注入化疗药物并全身静脉化疗 ,对照组单纯用全身静脉化疗。结果　观察组 30例中 ,完全缓解 (CR) 10例 ( 33.3% ) ,部分缓解 (PR) 17例 ( 5 6 .7% ) ,无变化 (NC) 3例( 10 % ) ,有效率 90 % ,明显高于对照组 ( 6 0 % ) ,P <0 .0 5。结论　经纤支镜局部化疗能较快地使气道内肿瘤缩小或消失 ,解除患者的呼吸困难 ;同时联合全身静脉化疗可明显缓解因肺癌原发灶、转移灶所引起的阻塞症状 ,此联合方法的客观疗效和症状缓解均优于单纯全身化疗

Therapeutic effect of local chemotherapy via fibrobronchoscopy on advanced brouhogenic carcinoma

Objective To investigate the therapeutic effect of local chemotherapy via fibrobronchoscopy on advanced bronchogenic carcinoma.Methods 60 patients with advanced bronchogenic carcinoma were randomly divided into observation group (n=30) and control group (n=30). The 30 patients in observation group were treated by local chemotherapy with Olympus BF 20 and its matched injecting needle. The chemotherapeutic drugs were injected into the center and surroundings of the tumor, combined with routine systemic chemotherapy. While the 30 patients in control group were treated only by routine systemic chemotherapy.Results In observation group, 10 patients (33.3%) completely recovered,17 patients (56.7%) partially recovered and 3 patients (10%) were not changed in their symptoms. The effective rate (90%) in observation group was significantly higher than that (60%) in control group (P<0.05).Conclusion The local chemotherapy via fibrobronchoscopy can obviously reduce the size of tumor and relieve the symptoms of dyspnea.Meanwhile systemic chemotherapy can alleviate the obstruction caused by tracheal or bronchial neoplasms. The combined therapy is much better in the aspects of therapeutic effect and symptomatic relief than routine systemic chemotherapy.
