关节镜下治疗髋关节结核的围手术期康复措施

目的探讨在关节镜辅助下对35例髋关节结核进行微创治疗围手术期的护理措施及康复要点。方法 2006年1月至2010年1月,关节镜下治疗髋关节结核共35例。男21例,女14例,年龄8~42岁,平均23岁,病史为3~10个月。所有患者术前明确诊断后均采用抗结核治疗,且血沉降至40 mm/h以下或呈明显下降趋势,在关节镜辅助下行髋关节镜探查、关节腔结核病灶清除术。术后行合理有效的护理及康复,同时遵循全身支持疗法和抗结核药物原则,促进伤口愈合及功能康复,最后做好出院指导及康复锻炼指导。结果所有患者在术后12个月门诊随访无关节结核复发,Harris评分平均为87.5分。通过关节镜手术治疗,患者关节功能均有很大程度的改善。结论髋关节结核是一种具有严重破坏性的疾病,在抗结核药物治疗下,通过关节镜下行髋关节结核病灶清除,创伤小、成功率高且基本无关节结核复发,另外,早期积极有效的围手术期护理及术后康复锻炼在髋关节结核关节镜术后关节功能恢复的过程中有着非常重要的作用。

Perioperative nursing care and rehabilitation for patients undergoing treatment of tuboculosis of hip joint by arthroscopy
