左旋肉毒碱(雷卡)在维持性血液透析肾性贫血治疗中的应用

摘    要:目的观察左旋肉毒碱在维持性血液透析肾性贫血治疗中的作用.方法将31例慢性肾衰竭维持性血液透析治疗的患者,随机分成治疗组及对照组,两组患者透析后皮下注射促红细胞生成素(济脉欣)3 000u,3次/周,口服铁剂(力蜚能)治疗,治疗组在透析后静脉注射左旋肉毒碱1g,3次/周,共12周.结果治疗组患者血Hb、Hct上升幅度明显高于对照组(P<0.05或P<0.01),治疗组血浆白蛋白、转铁蛋白较对照组显著增加,乏力、精神状况、食量、胸闷气短等临床症状较对照组明显改善(P<0.05).结论慢性肾衰竭维持性血液透析患者补充左旋肉毒碱可增加患者对促红细胞生成素的敏感性,提高其疗效,无明显不良反应.

Abstract:Objective To assess the efficacy of maintenance hemodialysis patients with anemia treated with L-carnitine.Methods Thirty-one maintenace hemodialysis patients randomly divided into 2 groups:treatment group and control group.Two groups all were treatet by rHuEPO(3000u,three times each week by subcutaneous injection) and oral Niferrix(polysaccharide-iron).In treatment group,1g of L-carnitine was infused after each hemodialysis treatment,three times each week,for 12 weeks.Results Compared with the control group,Hb,Hct were all significantly increased in treatment group,the serum albumin,transferring,level of the treatment group were elevated significantly(P<0.05 or P<0.01);The improvement of clinical symptoms of acratia,mental status,appetite,chest distress,short breath were obvious(P<0.05).Conclusion Intravenous treament of L-carnitine may improve the sensitive of EPO,is safe and effective in maintenance hemodialysis patients.
