| 摘 要： | 目的 探讨并比较经皮椎体成形术与椎体后凸成形术治疗骨质疏松性胸腰椎压缩性骨折的临床疗效.方法 将入院的58例患者随机分成两组,即A组35例采用经皮椎体成形术(PVP)治疗;B组23例采用经皮椎体后凸成形术(PKP)治疗,以比较两组手术前后的临床疗效、椎体前缘高度的恢复情况、并发症发生率.结果 经平均13个月的随访,A组术后椎体前缘平均高度达正常的(75.95±5.11)%.B组术后椎体前缘平均高度达正常的(94.74±4.05)%.临床疗效评定采用疼痛强度评价的视觉模拟评分(visual analogue scale,VAS),镇痛药使用评分,活动能力评分及WHO标准的疼痛缓解程度,经统计学分析两组间差异无统计学意义(P>0.05);每组内手术前后比较差异有统计学意义(P<0.05).术后并发症的发生率两组间差异无统计学意义(P>0.05).结论 使用PVP及PKP治疗胸腰椎压缩性骨折具有疗效优良、并发症少的优点.PKP组复位较好,可恢复病椎高度,但价格昂贵;PVP组操作更简便,疗效确实且价格便宜.
