| 摘 要： | 目的 探讨肝内胆管结石外科治疗的效果与方法.方法 对近13年来收治的198例肝内胆管结石的临床资料进行回顾性分析.结果 165例(83.3%)随访2～16年.20例(10.0%)发生术后并发症,2例死亡.肝叶切除术优良者57例(89.1%),残石复发者12例(l8.8%),非肝叶切除术优良者68例(67.3%),残石复发者40例(39.6%),两者优良率及复发率差异均有统计学意义.手术联合术中术后纤维胆道镜应用,残石率由常规手术组的31.5%降至8.8%;复发率由常规手术组的27.3%降至11.8%.结论 术前影像学全面地了解结石的分布和胆管狭窄的部位,恰当的肝部分切除,术中、术后应用胆道镜取石,能有效地减少术后并发症的发生,降低残石率和结石复发率,值得推广.
