浅快呼吸指数在机械通气撤机中的应用价值

目的 探讨浅快呼吸指数(RVR)对机械通气患者撒杌的预测价值.方法 62例准备撤机的患者按呼吸衰竭(呼衰)病因分为急性呼衰(ARF)组38例和慢性呼衰(CRF)组24例.采用低水平压力支持通气(PSV)模式进行撤机.在自主呼吸试验(SBT)开始3min时测定呼吸频率(f)、潮气量(Vt),并计算RVR值(f/Vt).比较撤机成功组和失败组RVR值及RVR预测撤机成功的敏感性、特异性和准确性.结果 ARF撤机成功组RVR值(62.3±18.5)bpm/L,明显低于ARF撤机失败组的(84.7±25.4)bpm/L(P<0.05);CRF撤机成功组RVR值(66.5±20.8)bpm/L,明显低于CRF撤机失败组的(93.2±27.6)bpm/L(P<0.05).以RVR值≤105bpm/L为标准,预测ARF组撤机成功的敏感性为93.8%、特异性为33.3%、准确性为84.2%;预测CRF组撤机成功的敏感性为89.5%、特异性为40.0%、准确性为79.2%.以RVR值≤125bpm/L为标准,预测ARF组撤机成功的敏感性为100%、特异性为16.7%、准确性为86.8%;预测CRF组撤机成功的敏感性为94.7%、特异性为20.0%、准确性为79.2%.结论 RVR测定简便、无创,预测的准确性较高,是一个较好的临床撤机指标.
