腰椎后路镜下椎间盘髓核摘除术标准程序化治疗腰椎间盘突出症

目的 探讨标准程序化腰椎后路镜下椎间盘髓核摘除术,提高腰椎间盘突出症手术治疗的临床效果.方法 制定腰椎间盘突出症术前及术后标准化的治疗与康复程序和后路镜下椎间盘髓核摘除术的标准化操作程序.应用这一标准化治疗程序,我院在2003年1月～2005年1月期间共治疗腰椎间盘突出症患者285例,男187例,女98例,平均年龄38.5岁.结果 208例获随访, 术后随访平均12个月,其中疗效为优者190例(91.4%),疗效为良者13例(6.2%),疗效为可者3例(1.4%),差2例(0.96),优良率达97.6%.结论 应用腰椎后路镜下椎间盘髓核摘除术标准程序化治疗腰椎间盘突出症,临床疗效的优良率高而疗效稳定.术前、术中及术后操作、治疗和康复的标准程序化,利于治疗的标准化和技术的准确掌握及正确应用,便于临床推广应用和常规化,以提高腰椎间盘突出症的整体手术治疗水平.

Research of standard programme of posterior microendoscopic discectomy in treatment of lumbar disc herniation

Objective To explore the clinic outcome of standard-programmed posterior microendoscopic discectomy(MED)in the treatment lumbar disc herniation.Methods To make up the standand programme for preoperative and postoperative treatment of lumbar disc herniation,and standard programmed management of posterior microendoscopic discectomy.To treat 285 cases of lumbar disc herniation with these standand programmes from January 2003 to January 2005.Among them,there were 187 males and 98 females with average age of 38.5 years.Results Two hundred eight cases were followed up for an average followed-up period of 12 months.The results were excellent in 190 cases(91.4%),good in 13 cases(6.2%),fair in 3 cases(1.4%) and poor in 2 cases(0.96%).Conclusion Standard programmed posterior microendoscopic discectomy for the lumbar disc herniation can get a good clinical outcomes and keep good curative effect.It has the advantages of minimal invasion,little blood loss and rapid recovery.It is an effective and safe method for preoperative,operative and postoperative treatment of lumbar disc herniation.
