河北省正常人群肺功能检测分析

摘    要:目的探讨河北省正常人群肺功能的生理预计值及其不同生活环境、年龄、身高、体重等因素与肺功能的关系.方法2002年8~10月对进行过基础体检并确定为正常的1 587名不同年龄段人群采用便携式肺功能仪进行肺容量及肺通气量检测,分析不同地域、年龄、性别、身高、体重等因素对肺功能的影响.结果检测的各项肺功能指标城乡之间差异无显著性(P>0.05);不同性别之间差异有显著性(P=0.000);多数肺功能指标在男12~16岁、女12~14岁为肺功能增长突增期,肺活量(VC)、用力呼气肺活量(FVC)、一秒量(FEV1)26岁达到高峰并维持到34岁左右,然后呈逐渐下降趋势;最高呼气流速(PEF)、25%用力呼气流速(FEF25%)可持续增长到34~44岁其后呈下降趋势(均P<0.01);FEF50%、FEF75%等18岁为高峰随后随年龄增长逐渐下降(P<0.01).身高、体重与各项肺功能指标均有相关性,但身高的影响大于体重.结论年龄、身高、体重与各项肺功能均有相关性,肺功能随年龄的增长而发生变化,青少年期、成年期、中老年期应有不同的生理预计值;本研究为正常人群肺功能值提供了一个参考值.

Abstract:Objective To investigate the lung function of the normal subjects living in Hebei province and its correlative factors such as living circumstance, age, height, and body weight. Methods The lung volumes and breath capacities of 1 587 normal subjects were tested by portable spirometers (Scope Rotry) from August to October in 2002. The influences of living circumstance, age, gender, height, and body weight on lung functions were observed and analyzed. Results No significant difference was found between urban and rural areas in all indexes (P > 0.05); however, significant difference existed between male and female subjects (P= 0.000). The change trends of lung function in male and female subjects were similar. Growth spurt appeared at the age of 12-16 years in male subjects and 12-14 years in female subjects. Vital capacity (VC), forced vital capacity (FVC), and forced expiratory volume in one second (FEV1) reached their peaks at the age of 26-34 years and then decreased with age. Peak expiratory flow (PEF), 25% forced expiratory flow (FEF50%), and 75% forced expiratory flow (FEF75%) appeared at the age of 18 and then went down with age. Both height and weight had a correlation with all the indexes of lung functions, although the influence of height is stronger than weight. Conclusions All the indexes of lung function have correlations with age, height, and weight. Lung function changes with aging, therefore different expected values shall be available for the adolescence, young adults, and middle-aged and old people.This study provides reference values of lung function for normal population.
