超声心动图对系统性红斑狼疮患者肺动脉压的初步研究

摘    要:目的应用超声心动图对系统性红斑狼疮(SLE)患者肺动脉压进行研究。方法通过对SLE组和正常对照组肺动脉血流频谱、肺动脉瓣口返流频谱及三尖瓣口返流频谱相关参数进行测量、计算、比较,观察组间肺动脉压变化情况。结果SLE组中71%的患者存在肺动脉高压,肺动脉血流加速时间(AT)较正常组显著缩短,右心室射血前期时间/肺动脉血流加速时间(PEP/AT)比值显著增大(P〈0.05),SLE组肺动脉收缩压(SPAP)及肺动脉平均压(MRAP)较正常组显著增高(P〈0.05)。结论SLE可以引起肺动脉高压,可通过观察肺动脉血流频谱形态及估测SPAP、MPAP判断SLE患者有无肺动脉高压及肺动脉高压的程度,为SLE患者临床治疗方案的拟订提供重要的参考信息。

Abstract:Objective To research pulmonary pressure in systemic lupus erythematosus(SLE).Methods Twenty-eight patients with SLE and 30 normal subjects as control group underwent transthoracic echocardiography.The pulmonary artery flow velocity curve,pulmonary artery regurgitation velocity curve and tricuspid regurgitation velocity curve were recorded.The relative parameters,such as AT,PEP,PEP/AT,SPAP,DPAP,MPAP were measured and calculated.The difference of those indexes were compared between the SLE group and control group.Results Twenty patients with SLE were proved to be pulmonary hypertention.Compared to control group,the AT of pulmonary artery flow velocity curve decreased significantly(P<0.05),while the value of PEP/AT,SPAP and MPAP increased significantly(P<0.05).Conclusion These parameters are useful in diagnosis of pulmonary hypertention with SLE.So it can provide some help for clinical treatment of SLE.
