危重监护室呼吸机相关性肺炎病原菌临床研究

摘    要:目的 通过对呼吸机相关性肺炎病原菌的构成及耐药情况的研究 ,为临床治疗提供依据。方法 急诊ICU室收治的86例机械通气患者 ,其中 6 4例发生呼吸机相关性肺炎 ,对其致病菌及药敏结果进行回顾性调查。 结果  6 4例检出 84株致病菌 ,其中革兰阴性细菌 (G-菌 )占 6 9.1% (5 8/ 84 ) ,革兰阳性细菌 (G 菌 )占 19.1% (16 / 84 ) ,真菌占 11.9% (10 / 84 ) ;前 4位致病菌分别为铜绿假单胞菌 (2 9株 ) ,金黄色葡萄球菌 (12株 )肺炎克雷白氏菌 (14株 )白色念球菌 (10株 )。药敏结果显示这类菌株耐药现象严重。 结论 呼吸机相关性肺炎病原菌以G-菌为主 ,呈多重耐药 ,临床上合理使用抗生素至关重要。

Abstract:Objective To analyze the variety of pathogens of lower respiratory tract infection in patients receiving mechanical ventilation and the microbial drug resistance . Methods There were 86 patients institutionalized in ICU which received mechanical ventilation and then pneumonia occurred in 64.The pathogens andtheir drug resistance were reviewed. Results A total of 84 strain pathogens were isolated by microbial culture. The ratio of gram-negative bacteria tototal microorganism isolated was 69.1%(58/84),and the gram-positive bacteria was 19.1%(16/84), and fungi were 11.9%(10/84). Among pathogens the most common were P.aeruginosa (29 strains), S.aureus(12 strains), P.klebsiella (14strain), and Candida albican (10 strains), their drug-resistance-multipleto antimicrobial agents is serious. Conclusion Gram-negative bacteria are the majority of the pathogens seperated from patients receiving mechanical ventilation of pneumonia in ICU.The pathogens show drug-resistance-mutiple to antimicrobial agents,and emphasis is on the rational use of antibiotics.
