髌韧带和髂胫束联合重建治疗膝前后交叉韧带同时损伤

摘    要:目的 探讨膝前后交叉韧带同时损伤手术重建方法及时机,评估髌韧带和髂胫束联合重建的临床疗效。方法 分别采用自体髌韧带中1/3组织(骨-髌韧带-骨,BPB)和髂胫束,一期等长重建膝前后交叉韧带同时损伤12例,术后进行有效膝关节功能锻炼。结果 平均随访15.6个月,按Lysholm膝关节评分标准,70-80分4例,60-70分5例,60分以下3例;患者自诉满意率75%。结论 采用髌韧带和髂胫束联合重建膝前后交叉韧带近期疗效较满意;早期诊断、早期治疗和术后正确康复锻炼有重要意义。

Abstract:Objective To explore the methods and opportunity of reconstruction of anterior and posterior cruciate ligaments and evaluate the clinical effects of reconstruction using patellar tendon and iliotibial tract.Methods Twelve patients with rupture of anterior and posterior cruciate ligaments were treated simultaneously with isometric reconstruction using BPB and iliotibial tract and dirigated effectively in the knees after operation.Results Following-up ranged from 5 to 48 months(average 15.6 months).According to the Lysholm knee scale score,scores were 70 to 80 in 4 patients,60 to 70 in 5 patients and below 60 in 3 patients.The satisfactory rate of the patients was 75%.Conclusion The early clinical effect of reconstruction of anterior and posterior cruciate ligaments using patellar tendon and iliotibial tract is satisfactory.Early diagnosis,early treatment and accurate rehabilitation are very important.
