不同类型过敏性紫癜患儿免疫功能的变化趋势及意义

目的 测定并分析T细胞亚群和免疫球蛋白在不同类型过敏性紫癜(Henoch-Schordein purpura,HSP)急性期患儿外周血中的变化趋势及其在HSP发病机制中的作用.方法 选取HSP患儿114例,符合美国风湿病协会的诊断分类标准,2周内未使用糖皮质激素及其他免疫抑制剂的初发病例,按临床表现类型分为5组:皮肤型组29例,关节型组26例,腹型组19例,肾型组13例,混合型组为27例.正常儿童作为对照组29例.CD3、CD4及CD8等T细胞亚群百分率用流式细胞仪检查;IgA、IgG、IgM及补体C3、C4含量用特定蛋白分析系统检查.结果 在不同类型HSP患儿急性期,T细胞亚群CD4百分率降低,CD8百分率增高,CD4/CD8比值降低(P<0.05).B细胞除皮肤型组外其他各组与对照组相比均增高(P<0.05);自然杀伤细胞在腹型和肾型组较对照组减低(P<0.05).与对照组相比,Igh在混合型组中增高,IgM在关节型和混合型组减低(P<0.05).结论 HSP患儿急性期存在T细胞亚群功能紊乱,免疫球蛋白分泌异常,IgA外周血中水平高低可能与多种因素有关.

Changes of immune function in children with different subtypes of Henoch-Schonlein purpura and its significance
