后腹腔镜单纯性肾囊肿去顶术185例报告

目的探讨后腹腔镜下单纯性肾囊肿去顶术的方法及疗效。方法在全麻下用镜体分离法建立后腹膜腔,定位并显露肾囊肿,距肾实质约5mm用电钩和（或）超声刀将囊壁切除去顶。结果手术均顺利完成,无中转开放手术,未发生严重并发症,手术时间30～70min,平均45min,术中出血量5～60mL,平均24mL,术后住院4～8d,平均5.5d。随访1～24个月,平均10个月,未见囊肿复发及残留。结论后腹腔镜下肾囊肿去顶术创伤小,术后恢复快,并发症少,是单纯性肾囊肿首选的治疗方法 。

Retroperitoneal laparoscopic unroofing of simply renal cyst(Report of 185 cases)

Objective To explore the method and efficacy of retroperitoneal laparoscopic of laparoscopic unroofing of simply renal cyst. Methods A total of 185 cases with renal cyst were enrolled in this study. Under general anesthesia, the retroperitoneal cavity was established by using laparoscope. The renal cysts were located and exposed. The cysts were unroofed at about 5mm from the edge of the renal parenchyma with electrical claw and/or harmonic scalpel. Results The operations were completed in all the cases without severe complications. The operation time was 30～70 min（mean 45 min）, blood loss was 5～60 mL （mean 24 mL）, and hospital stay was 4～8 d （mean 5.5d）. The 185 patients were followed up for 1～24 months （mean 10 months）, no recurrence and residual cyst was found. Conclusion The retroperitoneal laparoscopic unroofing of renal cysts has advantages of minimal trauma,rapid recovery and less complication, so it is the first choice for treatment of simply renal cyst.
