BiPAP无创通气治疗AECOPD合并Ⅱ型呼吸衰竭的疗效观察

目的]探讨应用双水平气道内正压（bi-level positive airway pressure,BiPAP）无创通气治疗慢性阻塞性肺疾病急性加重期（acute exacerbation of chronic obstructive pulmonary disease,AECOPD）合并Ⅱ型呼吸衰竭患者的临床疗效。方法]AECOPD合并Ⅱ型呼吸衰竭患者54例随机分为治疗组（28例）和对照组（26例）,对照组给予常规治疗,治疗组在常规治疗的基础上接受BiPAP无创通气治疗,分析两组血气（PH、PaCO2、PaO2）、呼吸频率（RR）、心率（HR）变化情况。结果]治疗组接受BiPAP无创通气治疗24h后、72h后血气分析,RR、HR改善明显,优于对照组,两组差异有统计学意义（P〈0.05）。结论]BiPAP无创通气治疗AECOPD合并Ⅱ型呼吸衰竭疗效满意,值得临床推广。

Clinical Effect Observation of BiPAP Noninvasive Ventilation in the Treatment of Acute Excerbation of Chronic Obstructive Pulmonary Disease with Type Ⅱ Respiratory Failure

Objective] To explore the curative effect of Bilevel Positive Airway Pressure（BiPAP） noninvasive ventilation in treating acute exacerbation of chronic obstructive pulmonary disease（AECOPD） with type Ⅱ respiratory failure.Methods] 54 patients with AECOPD with Type Ⅱ respiratory failure were randomly divided into experimental group（28） and control group（26）.The control group was treated with the standard therapy and the experimental group was treated with immediate BiPAP noninvasive ventilation mode based on the standard therapy.Comparison was then made for arterial blood gas analysis,respiratory rate,heart rate between the groups.Results] The blood PH,PaO2,PaCO2,respiratory rate,heart rate of the experimental group were obviously better than those of the control group after the treatment for 24 hours and 72 hours.The difference between the control and the experimental group was statistically significant.Conclusions]The clinical curative effect of BiPAP noninvasive ventilation in treatment of AECOPD with Type Ⅱrespiratory failure is significant and it is worth promoting.
