68例胫骨平台骨折的治疗

为探讨胫骨平台骨折的诊断及最佳的治疗方法 ,对 1 996年 1月至 2 0 0 3年 3月间首都医科大学宣武医院骨科收治的 68例胫骨平台骨折采用切开复位内固定及石膏外固定的方法治疗的患者进行回顾性研究。对所有患者随访 1 0～ 72个月 ,平均 2 3个月。结果 :按Merchant评分标准 ,膝关节功能优良率 89.7%。提示 :对移位及塌陷的胫骨平台骨折应采用切开复位内固定术 ,以恢复关节面的吻合关系和力线。同时对骨质缺损患者给予植骨。术后早期进行功能锻炼 ,恢复膝关节功能 ,减少并发症 ,是治疗的一种较好方法。

Treatment of 68 Cases with Tibial Plateau Fractures

The aim was to investigate dignosis and the best treatment technique for displaced and defective fractures of the tibal plateau by surgery. Treatment of sixty-eight patients with fracture of tibial plateaus fracture in 1996—2003 year were analyzed retrospectively. 68 cases were followed up ranging from 10～72 months, with an average of 23 months. The excellent and good rate was 89.7% on Merchant grading system. It suggests that it is optimal to perform open reduction and internal fixation for displace fracture of the tibial plateau, to support normal relation ship and good reduction by strong fixation, and allograft implantation for defective fracture. It is an effective way to move about knee joint early after operation to get the best healing function.
