2 型糖尿病合并不同分级高血压患者血清瘦素和 神经肽Y 的变化及影响因素分析

摘    要:目的 探讨血清中瘦素和神经肽Y 水平在2 型糖尿病合并不同分级高血压患者中的改变及临床意 义。方法 选取197 例2 型糖尿病患者作为研究对象,分为单纯2 型糖尿病组(DS 组)45 例、2 型糖尿病合 并不同分级高血压组(DH 组)152 例。DH 组包括2 型糖尿病合并高血压1 级(DH1 组)47 例,2 型糖尿病 合并高血压2 级(DH2 组)51 例,2 型糖尿病合并高血压3 级(DH3 组)54 例。另选取40 例健康者作为对 照组(NS 组)。应用酶联免疫吸附试验测定血清瘦素和神经肽Y 水平。多元线性回归法分析两者与各影响因 素的相关性。结果 DS 组瘦素和神经肽Y 水平较NS 组均升高,且随血压分级增加,DH1、DH2、DH3 组瘦 素和神经肽Y 水平均高于DS 组,均以DH3 组最高,差异有统计学意义(P <0.05)。瘦素和神经肽Y 均与收 缩压、空腹血糖呈正相关,进一步Logistic 回归分析提示收缩压和空腹血糖均是瘦素和神经肽Y 水平的影响 因素。结论 在2 型糖尿病合并高血压发生时,血清瘦素和神经肽Y 水平均升高,且随着血压分级的增加升高 明显,表明两者可能共同对2 型糖尿病合并高血压的发生有一定的影响。

Abstract:Objective To investigate the changes of serum leptin (LPN) and neuropeptide Y (NPY) in patients with type 2 diabetes and different grades of hypertension and its clinical value. Methods In this study, 197 patients with type 2 diabetes were selected and divided into 45 cases of simple diabetes (DS group) and 152 cases of type 2 diabetes with different grades of hypertension (DH group). The DH group included 47 cases of diabetic patients with grade 1 hypertension (DH1 group), 51 cases of diabetic patients with grade 2 hypertension (DH2 group) and 54 cases of diabetic patients with grade 3 hypertension (DH3 group). Meanwhile 40 healthy people were enrolled in this study as normal controls (NS group). The serum levels of LPN and NPY were detected by ELISA. The relationships of LPN and NPY with each influencing factor were analyzed by multiple linear regression. Results The levels of LPN and NPY in the DS group were higher than those in the NS group, at the same time, the levels of LPN and NPY in the DH1 and DH2 and DH3 groups were higher than those in the DS group, and were the highest in the DH3 group (P < 0.05). The result of the related analysis showed that LPN and NPY were positively correlated with SBP and FBG. Logistic regression analysis showed that SBP and FBG both were positively correlated with LPN and NPY levels. Conclusions Serum levels of LPN and NPY increase in patients with type 2 diabetes and hypertension. Both LPN and NPY might be closely related to the occurrence and development of diabetes and hypertension.
