中西医治疗原发性肝癌并门静脉癌栓69例临床观察

目的 观察超选择性肝段动脉内灌注化疗栓塞、经脾动脉间接门静脉化疗及中药艾迪注射液联合肝素抗凝综合治疗肝细胞癌(HCC)伴门静脉癌栓(PVTT)的疗效.方法 138例肝细胞癌合并门静脉癌栓患者随机分为对照组和综合治疗组各69例;分别进行单纯肝动脉内灌注化疗栓塞和超选择性肝段动脉内灌注化疗栓塞、经脾动脉间接门静脉化疗及中药艾迪注射液联合肝素抗凝等治疗.结果 综合治疗组有效率完全缓解+缓解(CR+PR)为71.0%(49/69),明显高于对照组40.6%(34/69);综合治疗组1、2、3年生存率分别为81.2%、62.3%和49.3%,中位生存期为17个月,而对照组分别为57.9%、37.3%和27.5%,中位生存期为7个月,两组比较差异有显著性(P<0.01),临床受益疗效综合治疗组明显高于对照组(P<0.05).结论 该技术能明显提高肝细胞癌合并门静脉癌栓患者近期治疗有效率和生存率,增强机体免疫力,提高生存质量,实为治疗HCC伴PVTT的理想新技术.
