幽门螺杆菌感染与冠心病发病关系探讨

目的：探讨幽门螺杆菌（Ｈｐ）感染与冠心病（ＣＨＤ）发病的关系及产细胞毒素Ｈｐ菌株对ＣＨＤ的影响。方法：采用成组病例对照研究。两组病例均采用间接酶标法（ＥＬＩＳＡ）检测血清Ｈｐ特异性抗体ＩｇＧ及产细胞毒素ＨｐＩｇＧ。结果：ＨｐＩｇＧ阳性率及ＨｐＩｇＧ平均浓度病例组与对照组比较有显著性差异；ＨｐＩｇＧ阳性与ＣＨＤ有相关关系，且在校正年龄因素后仍有相关关系。产细胞毒素ＨｐＩｇＧ阳性率两组比较无显著性差异，且它与ＣＨＤ无相关关系。ＣＨＤ组中检测抗ＨｐＩｇＧ阳性与阴性血脂和ＷＢＣ计数均无显著性差异。结论：Ｈｐ感染可能是ＣＨＤ发病的一个重要危险因素，但尚需前瞻性研究证实。

PRELIMINARY EXPLORATION OF THE ASSOCIATION BETWEEN HELIC OBACTER PYLORI INFECTION AND CORONARY HEART DISEASE

Objective To explore the association between Hp infection and CHD and to determine the effects of cytotoxic strains of Hp on CHD. Methods A nested case-control study. Cases are angiographically documented CHD patients and diagnosed acute myocardial infarction patients. Controls are patients without history of CDH matched for age, gender and occupation. Serum was analyzed for the presence of Hp specific IgG antibodies and cytotoxic strains of Hp specific IgG antibodies by ELISA. Results The proportion of Hp-positive individuals and the average concentration of Hp IgG were higher in the cases than the controls. The Hp seropositivity was related to CHD (OR=1.0389, P=0.018); The association was not altered after adjustment for age by multiple logistic regression (OR=1.040, P=0.019). No increase was found in cytotoxic strains of Hp seropositivity in cases compared with controls and the association was not found between cytotoxic stains of Hp seronegativity among cases about CHO, TG, HDL, WBC. Conclusions Hp infection may be an important risk factor for CHD. But it needs to be confirmed by more prospective studies.
