| 摘 要： | 目的分析胰十二指肠切除术后胰瘘的原因及预防经验.方法回顾分析1994～2003年行胰十二指肠切除术的63例患者的临床资料,对影响胰瘘发生的因素和采取的治疗方法进行总结.结果胰瘘与吻合技术有关;胰瘘的发生与残胰断面的处理有关;胰瘘的发生与胰实质的坚硬度、胰管的直径有关;胰管内支撑引流可预防PD术后胰瘘.结论胰管内支撑引流和良好的胰切面血供是预防pD术后胰瘘的关键.

| Abstract: | Objective: To study the prevention and reasons of pancreatic fistula after pancreaticoduodenectomy. Methods: A retrospective analysis was made on 63 patients with pancreaticoduodenectomy from 1994 to 2003, in order to evaluate the reasons of pancreatic fistula. Results: Occurrence of pancreatic fistula was related with pancreaticojejunal anastomosis; Occurrence of pancreatic fistula was related with manages of pancreatic section; Occurrence of pancreatic fistula was related with stiffiness of pancreatic parenchyma and pancreatic duct diameter; Pancreatic duct endoprosthetic drainage to prevent pancreatic fistula after pancreaticoduodenectomy. Conclusion: Pancreatic duct endoprosthetic drainage and nicer bloodstream purvey of pancreatic section are a key to prevent pancreatic fistula after pancreaticoduodenectomy. |
