老年骨质疏松患者股骨粗隆间骨折的内固定治疗

目的探讨不同固定方法早期手术治疗老年骨质疏松患者股骨粗隆间骨折的疗效.方法早期采用空心加压钉、DHs和Ender钉等不同固定方法治疗老年股骨粗隆间骨折78例.早期锻炼,延迟负重.结果随访75例(96.1%),随访时间6～42个月,平均22个月.围手术期无严重并发症发生,切口无感染,骨折均愈合,骨折愈合时间3～8个月,平均4.5个月.按临床综合评定标准评定:优66例(88%),良7例(9.3%),可2例(2.6%).结论根据老年患者骨质疏松情况和粗隆间骨折类型选择不同固定方法,合理锻炼,可有效减少各种并发症,降低伤残率.

Internal fixation for intertrochanteric fracture in old people with osteoporosis

Objective] Compare the effects of several kind of internal fixations for intertrochanteric fracture in old people with osteoporosis.Method] 78 old people with intertrochanteric fracture were treated by hollow-compression nail,DHS,Ender nail,early exercise,late weight bearing.Result] We followed 75 patients(96.1%)for 6-42 months,22 months average.There was no severe complications in perioperation,no infection.Fracture union in 3 to 8 months,4.5 months average.Outcomes of 66 patients is excellent,7 is good,2 is fine.Conclusion] According to general condition,degree of osteoporosis,and type of fracture,we should select different fixation method,exercise reasonable.Complication and invalidity rate will decrease.
