结石性胆囊炎与胆囊癌的临床研究

目的：探讨结石性胆囊炎与胆囊癌发生之间的关系。方法：总结手术切除胆囊结石4249例，合并胆囊癌23例的 理资料，以无症状性胆囊结石与症状性胆囊结石为两个研究单元，对比胆囊癌的发病情况及预后，结果：无症状性胆囊结石组发现Nevin I,II18例，III期1例，IV期2例，症状性胆囊结石发与NevinⅢ期胆囊癌2例，手术随访5年以上16例，5年生存率69．56％，结论：积极手术治疗，可有效防治胆囊癌。

STUDY THE RELATIONSHIP BETWEEN GALLSTONE AND PRIMARY CARCINOMA OF GALLBLADDER

0bjective:To investigate the relationship between gallstone and the development of gallbladder carcinoma. Methods: To clinic- pathological record of 4249 cases of gallstone underwent cholecystectomy from January 1982 to December 1999 in one hospital was retrospectively studied. Twenty- three cases of gallbladder carcinoma were found. The occurence of gallbladder carcinoma and its prognosiswere compared in the asymptomatic gallstone group with the symptomatic. Results: There were 18 cases of Nevin Ⅰ、 Ⅱ ,one case of NeivⅢ and 2 cases of Nevin Ⅳ gallbladder carcinoma in the asymptomatic group. Two cases of Nevin Ⅲ were found in the symptomatic group. Postoperatively all 23 patients who had different kinds of curative resection were followed-up for 14 months to 11 years, among them 16 cases were followed - up for more than 5 years. The 5 - year survival rate was 69.56 %. Conclusions: The therapeutic strategy for asymptomatic gallstone should be re- evaluated. Postitive attitude to surgical treatment of gallstone.
