重症脑梗死患者血浆D-二聚体测定的临床意义

目的探讨血浆D-二聚体水平对血栓形成性脑梗死(CI)患者并发出血性脑梗死(HI)、多脏器功能障碍(MODS)影响的临床意义。方法应用酶联免疫吸附试验双抗体夹心法,对66例CI患者及46例正常人血浆D-二聚体定量分析,对66例血浆D-二聚体水平不同程度持续增高CI患者HI、MODS发生率进行分析。结果CI患者病后第4天及第14天血浆D-二聚体水平明显高于正常人(P<0.01),CI患者急性期、亚急性期血浆D-二聚体水平越高,HI、MODS发生率越高(P<0.01)。结论CI患者急性期、亚急性期血浆D-二聚体水平持续增高,提示高凝状态是CI重要病理生理基础。CI患者急性期、亚急性期血浆D-二聚体水平与HI、MODS发生率呈正相关,提示检测D-二聚体水平,可为临床评估重症CI患者的病情、疗效及预后提供有价值依据。

Significance of the serum crosslinked D-dimer in patients with serious cerebral infarction

Objective] To explore the significance of the effect of serum crosslinked D-dimer (XDP) on the cerebral infarction(CI) with hemorrhagic CI(HI) and multiple organ dysfunction syndrome(MODS). Mothods] We analysed the levels of serum XDP between the 66 CI and 46 normal, and the rate of CI with HI and CI with MODS among the 66 CI having diffent levels of XDP. Result] After the 4 days and 14 days of onset,the levels of XDP in CI was higher than that of the normal (P <0.01). Among the 66 CI, the higher the levels of XDP at the acute and subacute phases of CI was, the more the rate of HI and MODS was (P <0.01). Conclusion] The high levels of XDP at the acute and subacute phases of CI, point out that the state of high coaguiation is an important foundation of pathology in CI. The levels of XDP in CI is closely related to the HI, MODS. So it can evaluate the condition, curative effect and prognosis of serious CI.
