妊高征患者子宫胎盘床螺旋动脉的彩色多普勒超声检测

目的 :探讨妊高征患者子宫胎盘床螺旋动脉的血流动力学改变。方法 :应用彩色多普勒超声对 14对按 1∶1配对的妊高征组和正常妊娠组共 2 8例孕妇 ,在未行任何治疗前 ,检测子宫动脉及子宫胎盘床中央及边缘的螺旋动脉的PI、RI ,并动态监测至分娩。结果 :妊高征组子宫动脉、螺旋动脉的PI、RI值明显高于正常妊娠组 ,并以螺旋动脉更明显 ,P <0 .0 1。子宫胎盘床边缘的螺旋动脉RI、PI无论在妊高征组还是在正常妊娠组都高于子宫胎盘床中央的螺旋动脉RI、PI。结论 :彩色多普勒超声检测子宫胎盘床螺旋动脉血流状况对妊高征的发生发展有预测作用 ,并优越于子宫动脉。

COLOR DOPPLER FLOW IMAGING STUDY OF PLACENTAL BED SPIRAL ARTERIES IN PREGNANCY INDUCED HYPERTENSION

Objective:To determine the hemodynamic changes of the placental bed spiral arteries in patients with pregnancy induced hypertension (PIH).Methods:Fourteen women with PIH were matched 14 normotensive controls.We made colour doppler assessment of blood flow in the uterine arteries and spiral arteries in central and peripheral part of the placental bed. Impedance to blood flow in the spiral arteries in PIH was compared with which in normotensive controls. Results:In women with PIH there was significantly higher impedance to blood flow in spiral arteries compared with normotensive women.Conclusions: Ultrasound colour doppler based investigation of blood flow in spiral arteries in ongoing pregnancy can predict the progress and prognosis of PIH,and it is better than uterine arteries.
