10 kg以下婴幼儿心脏手术的围术期管理

目的 总结10kg以下婴幼儿心脏直视手术的围术期管理体会.方法 回顾性分析该院2001年3月～2004年5月收治的122例经历心脏直视手术的10kg以下婴幼儿的临床资料,探讨其术前准备、术中管理和术后监护的特点并总结一些经验教训.结果 该组患儿死亡6例,死亡率4.92%.术后出现并发症的16例,并发症发生率13.1%,经治疗后均康复出院,出院患儿恢复良好.结论 尽管10 kg以下婴幼儿心脏直视手术的并发症发生率和死亡率较高,但加强围术期的管理将极大提高低体重婴幼儿心脏直视手术的疗效和安全性.

Perioperative management of open heart surgery in infants below 10 kg

Objective] To generalize the experience of perioperative management of open heart surgery in infants below 10 kg. Methods] The clinical data of infants below 10 kg who were admitted in our hospital between 2001.3-2004.5 and underwent open heart surgery were analyzed retrospectively, and the features of preoperative preparation, management during operation and intensive care after surgery in infants were discussed also. Results] There were 6 deaths after surgery and the mortality was 4.92%. Complications were present in 16 cases and the incidence of complication is 13.1%, but they were asymptomatic and discharged uneventfully through careful treatment. All discharged patients recovered very well. Conclusions] Strengthening perioperative management will greatly improve the curative effect and safety of cardiac surgery in infants, though the mortality and the incidence of complication are relatively high.
