肝硬化患者门静脉、脾静脉血流动力学检测及临床意义

摘    要:目的 :了解肝硬化患者门静脉、脾静脉血流动力学状态与肝功能Child -Pugh分级的关系。方法 :应用彩色多普勒超声技术检测了 65例肝硬化患者 (A组 1 3例 ,B组 2 2例 ,C级 30例 )和 2 4例正常人的门静脉内经(Dpv)、门静脉血流速度 (Vpv)、门静脉血流量 (Qpv)、脾静脉内经 (Dsv)、脾静脉血流速度 (Vsv)及脾静脉血流量 (Qsv)。结果 :A ,B ,C级肝硬化患者Dpv,Dsv按A ,B和C顺序逐渐增宽 (P <0 .0 5) ,Vpv ,Vsv逐渐减慢 (P<0 .0 5) ,Qsv及Qsv/Qpv值明显加大 (P <0 .0 1 ) ,Qpv无明显变化 (P >0 .0 5)。结论 :多普勒彩色超声检测门静脉、脾静脉血流动力学有助于评价肝硬化患者的肝储备功能、门静脉高压程度及预后。

Abstract:Objective:To observe and analyse the relationship between the changes of the portal system hemodynamics and the Child-Pugh grade. Methods:The diameter of portal vein (Dpv) and splenic vein (Dsv), the blood flow velocity of the portal vein (Vpv) and the splenic vein (Vsv), the blood flow volume of the portal vein (Qpv) and splenic vein (Qsv) were examined in 65 patients with cirrhosis and 24 normal subjects by color ultrasonography. Results:The Dpv and Dsv were significantly wider ( P <0.05), the Vpv and Vsv were significantly slower ( P <0.05),the Qvs and Qsv/Qpv were significantly increased ( P <0.01) in Child-Pugh grade A,B and C group patients, successively. But the Qpv were not significantly changed in all groups ( P >0.05). Conclusions:Examination of portal system hemodynamics in patients with cirrhosis can be used to evaluate the degree of liver function and portal hypertension.
