灌注超声导管输出能量及其损伤效应的实验研究

摘    要:目的 :探讨一种新型灌注超声导管输出能量、消融时间与消融结果之间的关系及其用于临床心律失常治疗的可行性。方法 :8只健康成年杂种犬全麻后通气支持下开胸制作心包吊蓝及暴露双侧股方肌。消融采用灌注超声导管。心脏消融点沿前降支排列 ,股方肌消融点沿肌束方向排列。输出功率与时间排列组合后各有 16个消融点 ,以探讨不同消融能量与时间组合后对股方肌及心室肌损伤的程度。结果 :超声导管对组织的损伤主要引起灶性坏死。心室肌消融时损伤范围与输出功率成正相关 ,与输出时间无关。输出功率为 2 0W时 ,损伤范围明显较其它输出功率组间小 ;输出功率为 3 0W、40W时 ,在各输出时间组 ,二者对组织的损伤无明显差异 ;输出功率增加到 5 0W后 ,在各输出时间组均可导致较大范围损伤 ,且与其它输出功率组间有显著差异。在股方肌损伤范围同样与输出功率相关 ,与输出时间无关。结论 :研究证实输出功率在 3 0W和 40W时 ,心室肌和股方肌损伤范围稳定可靠 ,输出功率高于或低于此值损伤范围均明显增加或减少。在输出功率不变时 ,损伤范围与输出时间无关。

Abstract:Objective:To evaluate whether reliable lesions could be produced with ablation by a newly-designed, irrigated ultrasound catheter, and determine the optimal energy output and the duration of energy discharging. Methods:Eight adult canines under general anesthesia were included in present study. A left fourth intercostal space thoarecotomy was performed, pericardium was incised and the he art was suspended in a pericardial cradle. The myocardium between left descending artery and left circumflex artery were clearly displayed. Dradratus femoris was also exposed with excision of skin and subcutaneous tissue. Irrigated ultrasound catheter was applied to ablate both of the organs. The energy output and discharging time were integrated with each other, so for each of the organs, 16 sites were to be ablated.Results:Focal necrosis was the pathological changes after the ablation procedure. For LV myocardium, produced lesion was positively correlated with energy output, and not correlated with duration of energy discharging. Same results were also observed after ablating dradratus femoris, produced lesion was positively correlated with energy output, and not correlated with duration of discharging. Conclusion:Reliable lesions could be produced with the newly-designed, irrigated ultrasound catheter, 30W and 40W are the recommended energy output for further study.
