颞骨内面神经各段的实际测量与利用高分辨率CT测量的比较研究

摘    要:目的比较颞骨内面神经不同分段的长度及成角的实际测量值与利用高分辨率CT所得测量值差异,为临床应用提供解剖学基础。方法高分辨率CT测量是通过对标本进行常规高分辨率CT扫描后将扫描结果输入CT三维重建工作站进行的测量。实际测量是通过对标本颞骨内面神经各段进行具体解剖测量。结果颞骨薄层高分辨率CT测量结果:面神经的内听道段长度为11.7mm±1.2mm;迷路段长度为3.8mm±0.5mm;鼓室段长度为11.4mm±0.4mm;乳突段长度为11.6mm±1.0mm。颞骨标本上解剖学测量结果:面神经的内听道段长度为11.7mm±2.2mm,迷路段长度为3.5mm±0.5mm,鼓室段长度为11.0mm±0.9mm,乳突段长度为12.2mm±1.3mm。面神经第一膝CT测量值为62.9°±14.2°,第二膝为125.6°±14.4°。经解剖得到的面神经第一膝实际测量值为60.4°±7.8°,第二膝为122.7°±11.4°。统计学分析结果显示面神经段长度及成角CT测量值与解剖实际测量值经配对t检验未见显著性差异(P〉0.05)。结论颞骨内面神经内听道段、迷路段、鼓室段、乳突段及第一膝和第二膝角度实际测量值与高分辨率CT测量值无明显差异。

Abstract:Objective To compare different section of temporal bone concave nerve angles and the actual length measurement and high-resolution CT in clinical applications for the measured values,and to provide the anatomical basis.Methods The measurement of High-Resolution CT is through to routine CT scanning for samples after establishing three dimensional reconstruction workstation.Actual measurement is the operation procedures of temporal bone concave neural anatomy of each to concrete measurement.Results The thin layer temporal resolution CT measurement results were as follows: In the facial nerve,the internal acoustic meatus paragraph length was 11.7mm± 1.2 mm;the labyrinth paragraph length was 3.8mm± 0.5 mm;the tympanic cavity paragraph length was 11.4mm± 0.4 mm and the mastoid process paragraph length was 11.6mm± 1.0 mm.The temporal bone specimens of measurement results were as follows: In the facial nerve,the internal acoustic meatus paragraph length was 11.7mm± 0.4 mm;the labyrinth paragraph length was 3.5mm± 0.5 mm;the tympanic cavity paragraph length was 11.0mm± 0.9 mm;the mastoid process paragraph length was 12.2mm± 1.3mm.The first knee of facial nerve of CT measured value was 62.90°±14.2°;The second knee was 122.7°±11.4°;The anatomy of the actual measurement obtained for the first knee of the facial nerve was 66.40°±7.8°;The second knee was 122.7°±11.4°.Statistical analysis showed that the facial nerve length and angle which measured on CT with anatomical actual measurement,the rest of the matching T-test had not show significant different(P 0.05).Conclusion The internal acoustic meatus paragraph,the labyrinth paragraph,the tympanic cavity paragraph,the mastoid process and the angle of the first knee,second knee in temporal bone facial nerve,those of actual measurement value have no significant different with the angle of high-resolution CT measurement.In conclusion,the facial nerve' s measure value of the temporal bone CT can take the actual temporal concave neural paragraphs measurement for properly predicting the distance between facial nerve and adjacent anatomic structure would be of great significant.
