脑出血急性期血压水平对血肿扩大及预后的影响

目的 探讨脑出血急性期血压水平对血肿扩大及预后的影响.方法 将105例脑出血患者根据入院时血压分为收缩压升高组(SBP>140 mm Hg,68例)、收缩压未升高组(SBP≤140 mm Hg,37例)、舒张压升高组(DBP>90 mm Hg,76例)、舒张压未升高组(DBP≤90 mm Hg,29例),对4组患者血压与血肿扩大以及死亡之间的关系进行分析.结果 SBP/DBP升高组,年龄>60岁者比例及美国国立卫生研究院卒中量表(NIHSS)评分均明显高于SBP/DBP未升高者(P<0.05).SBP升高患者组发生血肿扩大的比例(35.3%,24/68)以及30 d内患者病死率(29.4%,20/68),均高于SBP未升高组(P<0.05).结论 脑出血急性期血压升高患者病情较未升高者严重,发生血肿扩大和预后不良者比例较高,临床应予以高度重视.

The effect of blood pressure levels on the enlargement of hematoma and prognosis in acute cerebral hemorrhage

Objective To investigate the effect of blood pressure levels on the enlargement of hematoma and prognosis in acute stage of cerebral hemorrhage.Methods According to the blood pressure in admission,105 cases of cerebral hemorrhage patients were divided into systolic blood pressure increased group(SBP>140 mm Hg,68 cases),systolic pressure dis-increased group(SBP≤140 mm Hg,37 cases),diastolic blood pressure increased group (DBP>90 mm Hg,76 cases),and blood pressure dis-increased group(DBP≤90 mm Hg group,29 cases),the relationship of patients blood pressure with hematoma enlargement and death rate were analyzed.Results In SBP/DBP increased group,the proportion of patients with age>60 years and NIHSS score were significantly higher than which in SBP/DBP dis-increased group(P<0.05);the proportion of hematoma enlargement occurred(35.3%,24/68)and mortality within 30 days(29.4%,20/68)were also higher than which in SBP dis-increase group(P<0.05).Conclusion For patients with acute cerebral hemorrhage,the illness is severer in patients with blood pressure increased,the incidence of hematoma enlargement and poor prognosis is higher,clinical should pay more attention.
