肱骨距在肱骨近端骨折手术治疗中的重要性分析

目的：探讨肱骨距在肱骨近端骨折手术治疗中的作用及术后临床疗效。方法回顾性分析2011年9月—2015年6月我院收治的25例肱骨近端骨折患者的临床资料，按照Neer分型，Ⅱ型13例、Ⅲ型12例，伴肩关节脱位2例，均采用肩关节前侧手术入路锁定钢板内固定治疗，术后3天肩关节被动活动锻炼，术后2周钟摆式锻炼，术后3周肩关节主动锻炼，并采用Neer肩关节功能评分评估恢复效果。结果所有患者手术切口均Ⅰ期愈合，并得到随访，随访时间6个月~15个月，平均11个月；Neer肩关节功能评分：优12例，良9例，可3例，差1例。结论肱骨距在肱骨近端骨折手术中可成为解剖复位标志，可在术后早期行康复锻炼，有利于肩关节功能的恢复。

The important analysis of humeral calcar in proximal humeral fractures during surgical treatment

ObjectiveExplore the humeral calcar is apart from the role in the surgical treatment of proximal humeral fractures and postoperative clinical curative effect.MethodsTwenty-five cases in our hospital suffered from proximal fractures were included in the study,according to the Neer classification,there were 13 cases of type Neer two,12cases of type Neer three,with to shoulder dislocation,through traditional pectoralis major-deltiod muscle approach,using locking compression plate to treat proximal humeral fractures. They were required to exercise the shoulder joint passively after 3 days postoperatively,then start to do the shoulder swing exercise 2 weeks after operation,try the lift,abduction,posterior extension and flexion exercise 3 weeeks after operation.Then use Neer rating scale.ResultsThe operation incision of the patients got healed in the first period.All the patients were followed up from 6 to 15 months,averagely 11months.For the Neer score:there are 12 cases of excellence,10 cases of good,6 cases of fair,1 case of bad.ConclusionsHumeral calcar will be sign in operation reduction of the proximal humeral fracture,early exercise,in order to recover the shoulder function.
