手动静脉推注大剂量药物的技术改进

目的：探讨更为高效、安全的手动静脉推注大剂量药物操作方法。方法783例用76%复方泛影葡胺进行CT增强扫描患者，其中631例行大角度静脉穿刺、加热并推注对比剂、妥善固定针头和拔针后大面积压迫止血等改进措施，统计注射速度、患者不良反应及处理、CT扫描增强效果，并与常规方法注射的152例进行比较，总结并优化其技术方法和经验。结果改进组穿刺点皮下瘀血和注射漏液的病例数显著少于常规组，注速≥1.5 mL/s和扫描部位近心端大动脉CT值≥150 Hu的病例数多于常规组，尤以使用直接针头注射者效果最佳；发生高渗性不良反应的病例数较多。结论改进后的手动静脉推注大剂量药物速度快、CT增强效果好，患者不良反应少。

Improved intravenous injection of large dose drug with manual push

Objective The paper studied effective and safe operation experience of intravenous injection of large dose drug with manual push. Method To improve intravenous injection at large angle venipuncture,heated contrast medium,properly fixed needles and large area oppression hemostasis after Pull out the needle for 631 cases in 783 cases with 76%meglumine diatrizoate CT enhanced examine.To statistics the number of cases in injection rates ,patient＆#39;s adverse reactions and processing,CT enhancement effects. To compare with 152 cases used routine method By χ2 test, and to optimize technical method of intravenous injection. Results The numbers of cases that occurred subcutaneous ecchymosis around puncture point and injection liquid leakage in improved group were less than notably routine group. The number of cases that injection speed≥1.5 mL/s and aortic CT value≥150 Hu in scanning field in their were more than routine,and effects by directive needle injection were best;The numbers of cases of hyper tonicitis adverse reactions were more. Conclusion The rate were fast with Improved intravenous injection of large dose drug by manual push,CT enhancement effect were better,and adverse reactions were less.
