电视腹腔镜阑尾切除手术中两种阑尾根部结扎法的比较

摘    要:目的:比较腹腔镜阑尾切除术时行阑尾根部结扎的两种不同方法的优缺点。方法:回顾性分析2007年1月至2008年2月我科213例腹腔镜阑尾切除术资料,分为推结器结扎组101例,直接结扎组112例,分别使用推结器与直接使用可吸收线结扎阑尾根部,比较其治疗效果。结果:213例全部顺利完成,无需中转开腹。推结器组与直接结扎组的平均手术时间分别为(42.73±5.68)min和(46.09±7.12)min(P〈0.01),平均术后第一次肛门排便或者排气时间分别为(26.74±3.77)h和(26.59±3.70)h(P〉0.05),平均住院天数分别为(3.83±0.93)d和(3.88±0.85)d(P〉0.05),两组病例手术时间差异有显著性意义,平均术后第一次肛门排便或者排气时间、平均住院天数比较差异无显著性意义。结论:腹腔镜阑尾切除术使用推结器结扎阑尾根部手术时间短、操作简单、术后恢复快,与使用可吸收线直接结扎同样安全。

Comparative research in two methods of the ligation of the base of the appendix in the laparoscopic appendectomy

Abstract:Objective To compare the effects of the using knot guide and knoting directly in the ligation at the base of the appendix in the treatment of the laparoscopic appendectomy. Methods In this retrospective study from January 1, 2007 to February 29, 2008, data of 213 patients with laparoscopic appendectomy in our hospital, oonsisit of knot guide group (101 patients) and knoting directly group (112 patients ) . Results Laparoscopic appendectomy was performed successfully in all patients, there was no conversion to open surgery. The operative time of knot guide group and knoting directly group was respectively (42.73 ± 5.68) min and (46.09 ± 7.12) rain ( P 〈 0.01 ), the time of began to pass flatus was (26.74 ± 3.77) h and (26.59±3.70) h (P 〉0.05), the postoperative stay was (3.83±0.93) d和(3.88±0.85) d (P 〉0.05) . Conclusions Using knot guide has the advantage of faster, more easy , and as safe as knoting directly in the ligation at the base of the appendix in the laparoscopic appendicectomy.
