子宫内膜癌的病理与超声对比分析及手术分期的相关性研究

目的探讨子宫内膜癌的病理分型与声像图表现及手术分期的相关性。方法本组78例患者,全部行诊断性刮宫病理提示为子宫内膜癌,其中Ⅰ型内膜癌69例,Ⅱ型内膜癌9例,超声检查测量宫腔回声厚度,观察宫颈结构及盆腔淋巴结,将两组不同类型内膜癌声像图表现与手术分期进行对比分析。结果Ⅰ型和Ⅱ型子宫内膜癌的发病年龄差异有统计学意义(P<0.05),但宫腔回声厚度无统计学意义(P>0.05)。结论子宫内膜癌的病理分型与超声图像的对比分析和手术分期具有相关性,能为临床治疗提供更多的循征依据。

The relationship between the contrastive analysis of pathology and sonogram of endometrial carcinoma and the surgical staging

Objective To study the associativity hetweem pathological type of endometrial carcinoma, ultrasonographic manifestations and surgical staging. Methods 78 cases in the study group were all endometrial carcinoma confirmed by pathological examination with diagnostic curettage. Thickness of uterine cavity, cervical structures and pelvic lymph nodes were observed with ultrasonography in all cases, of which Type I endometrial carcinoma in 69 cases and Type Ⅱ endometrial carcinoma in 9 cases. Comparative analysis of ultrasonographic findings and surgical staging were carried out in types of endometrial cancer. Results There were significant differences between type I and type Ⅱ patients in age of onset （P〈O. 05）, but there were no differences between two types in echo-thickness of cavitary uteri （P〉O. 05）. Surgical staging： staging Ⅰ , 58 cases accounted for 74.35% （58/78）; staging Ⅱ, 12 cases accounted for 15.38%（12/78）; staging Ⅲ, 5 cases accounted for 6.41% （5/78） ; staging Ⅳ, 3 cases accounted for 3.8% （3/78）. Conclusion Good correlations were observed between comparative analysis of the pathological type and ultrasound images in patients with endometrial carcinoma and surgical staging of endometrial cancer, which could provide more evidence-basis for clinical treatment.
