小探头超声内镜对上消化道隆起性病变的诊断和治疗价值

目的探讨小探头超声内镜对上消化道隆起性病变的诊断和治疗价值。方法对67例内镜诊断为上消化道隆起性病变的患者进行小探头超声内镜检查,根据检查结果,选择内镜下粘膜切除术(EMR)、粘膜下剥离术(ESD)、内镜粘膜下挖除术(ESE)或外科手术切除病变。结果 67例患者中,平滑肌瘤34例,息肉18例,异位胰腺7例,脂肪瘤3例,间质瘤2例,孤立性静脉瘤2例,囊肿1例;病变在胃镜下切除46例,抽取囊液1例,外科手术切除2例。结论小探头超声内镜对上消化道隆起性病变有较高的诊断价值,对病变进行内镜下治疗有指导意义。

A preliminary study of endoscopic miniprobe ultrasonography in the diagnosis and therapy of protuberant lesions in upper gastrointestinal tract

Objective To explore the value of endoscopic miniprobe ultrasonography in diagnosis and therapy of protuberant lesions in upper gastrointestinal tract.Methods Endoscopic miniprobe ultrasonography was conducted on 67 patients diagnosed protuberant lesions in upper gastrointestinal tract by gastroscope,and EMR,ESD,ESE or surgery was selected to remove the lesions according to the results of endoscopic miniprobe ultrasonography.Results The diagnosises of 67 cases were conformed,and there were 34 cases of leiomyoma,18 cases of polyps,7 cases of heterotopic pancreas,3 cases of lipoma,2 cases of stromal tumors,2 cases of solitary phlebangioma and 1 case of cyst respectively.The lesions of 46 cases were removed by gastroscope,2 cases by surgery,and the fluid of cyst was extracted by gastroscope in 1 case.Conclusion The higher diagnostic value of endoscopic miniprobe ultrasonography for protuberant lesions in upper gastrointestinal tract and the guiding significance of treatment by gastroscopy were confirmed.
