体外高频热疗联合腹腔热灌注化疗治疗恶性腹水41例

目的观察体外高频热疗联合腹腔热灌注化疗治疗恶性腹水的疗效及副反应。方法恶性腹水44例，同期分为治疗组及对照组。腹腔穿刺引流干净腹水后，治疗组22例给予43-45℃的生理盐水1000ml＋顺铂80mg／m2、生理盐水500ml＋5-氟尿嘧啶500mg／mz，以HGG2-102体腔热灌注治疗机（珠海和佳生产）快速灌注入腹腔，使进入腹腔的液体温度维持在41～43℃；灌注结束后，采用HG-2000Ⅲ型高频热疗机（珠海和佳生产）行腹腔热疗41～43℃，60分钟，每周热疗2次。对照组给予生理盐水1000ml＋顺铂80mg／m2、生理盐水500ml＋5-氟尿嘧啶500mg／m2腹腔内注入。两组均于1小时内完成，每周灌注1次，连续灌注2周，评定疗效。结果治疗组：有效率72．7％，KPS（一般状况）评分升高率54．5％；对照组有效率45．5％，KPS升高率31．8％。治疗组腹水控制率、KPS升高率均较对照组明显为优（P〈0．05）。两组副反应无明显差异。结论体外高频热疗联合腹腔热灌注化疗治疗恶性腹水疗效好，可以耐受。

Treatment of malignant ascites with hyperthermic perfusion chemotherapy and high-frequency hyperthermia

Objective To observe the efficacy and side effects of high-frequency hyperthermia and hyperthermie perfusion chemotherapy on malignant ascites. Methods 44 patients with malignant ascites were divided into treatment group and control group, respectively. In treatment group,after malignant ascites was drained, normal saline （1000ml） with cisplatin （80mg/m2） and normal saline （500ml） with fluorouracil （500mg/m2） （43 -45 ℃ ） were perfused into peri- toneal cavity and maintained at 41-43 ℃ with HGG2-102 body cavity perfusion therapy machine. In control group, patients only were given normal saline （1000ml） with cisplatin （80rag/m2） and normal saline （500ml） with fluorouraeil （500mg/m2）. Results The effective rate was 72.7 %, and KPS （general condition） score increase rate was 54.5 % in the treatment group,while 45.5%,31.8% in the control group,respectively. Conclusion High-frequency hyperthermia com- bined with hyperthermie perfusion chemotherapy in the treatment of malignant ascites have good efficacy.
