剖宫产后腹壁切口子宫内膜异位的超声表现及鉴别

目的探讨剖宫产后腹壁切口处子宫内膜异位的超声图像特征。方法对高频彩超检查诊断的38例腹壁切口子宫内膜异位症的患者与术后病理结果进行对比分析,以评价超声的诊断价值。结果超声诊断的38例腹壁切口子宫内膜异位症患者手术后病理诊断34例,超声正确诊断率89.47%(34/38),超声误诊4例,误诊率10.53%,分别为切口缝线炎性肉芽肿2例,切口放射状疤痕组织纤维增生1例,陈旧性血肿1例。结论高频彩超对腹壁切口子宫内膜异位症诊断准确率较高,且安全、无创,重复性好,对临床有较好指导价值。但应与陈旧性疤痕、陈旧性血肿及炎性反应鉴别,以减少超声的漏误诊率。

Image manifestation and identification of high frequency ultrasonography for endometriosis at abdominal incision after uterine-incision delivery

Objective To discuss image manifestation and of high frequency ultrasonography(HFU) in diagnosing endometriosis at abdominal incision after uterine-incision delivery.Methods After diagnosed by HFU.38 patients of endometriosis at abdominal incision after uterine-incision delivery were observed and treated and traced the pathologic correlation.Results Among the 38 cases of endometuiosis at abdominal incision diagnosed by HFU.four cases were misdiagnosed.The diagnostic accuracy rate of ultrasound was 89.47%.The misdiagnosis cases were inflammatory granuloma(2 case).scar fiber hyperplasy(1 case).old hematoma(1case).Conclusion To diagnose endometriosis at abdominal incision with HFU has important clinical value due to its noninvasive,convenience,safety and repeatability. The inflammatory granuloma.scar fiber hyperplasy and old hematoma ought to been identified.
