ACEI和ARB联合多巴胺治疗心肾综合征合并利尿剂抵抗的临床分析

目的探讨ACEI和ARB联合多巴胺治疗心肾综合征合并利尿剂抵抗的标准及方法。方法收集100例慢性心力衰竭合并肾功能不全及利尿剂抵抗（心功能Ⅲ-Ⅳ级,肌酐〈3mg/dl）的病人资料,并随机分为ACEI、ARB联合多巴胺治疗组和对照组。全部患者在入院时均进行心脏超声、胸片、肾功能、心电图等检查。两组病人的发病年龄、心功能分级、肾功能水平、浮肿及少尿程度无统计学显著差异（P〉0.05）。结果 ACEI和ARB联合多巴胺治疗可显著改善患者症状,使左室射血分数增加,体重降低,尿量增加。结论 ACEI和ARB联合多巴胺治疗心肾综合征合并利尿剂抵抗,无论对心功能的改善抑或肾功能的改善都是十分有益的。

Clinical analysis of ACEI and ARB complicated with dopamine for cardiorenal syndrome with diuretic resistance

Objective The purpose of the research was to study the standard and method of ACEI and ARB complicated with dopamine for cardiorenal syndrome with diuretic resistance.Method 100 patients with cardiorenal syndrome with diuretic resistance（heart function Ⅲ-Ⅳgrade） were randomly divided into ACEI and ARB complicated with dopamine treatment group and contrast group.There was no significant difference about ages,heart function,kidney function and dropsy in the two groups.Result ACEI and ARB complicated with dopamine to cure cardiorenal syndrome with diuretic resistance could improve clinical symptoms,heart and kidney function.Conclusion ACEI and ARB complicated with dopamine to cure cardiorenal syndrome with diuretic resistance can improve heart function and kidney function.
