杭州市院前急救中精神病例的救治现状及应对策略

目的：了解杭州市院前急救中精神病例的流行病学特点，分析救治过程中存在的不足，并提出应对策略。方法：以杭州市急救中心2011年的急救档案数据为基础，结合资深急救医师及急救管理者经验，阐述本文。结果：杭州市急救中心2011年院前急救精神病例为293人次，占总接警人数的O．48％，男性略多于女性；第四季度最多，占34．1％；年龄以中青年为主。现场滞留时间久，现场类型以家属报警为主，常需要家属及民警协助采取强制措施，医疗急救干预较少。结论：杭州市院前急救处置精神病例中，存在急救人员疾病诊断能力不足等问题。通过完善处置流程、使用保护性约束、加强精神专科档案管理等举措，可进一步保障精神病人及其他急救病人的利益。

Medical Treatment Status and Strategies of Mental Cases in the Pre - hospital Emergency Medical Care in Hangzhou

Objective ： To understand the epidemiologieal characteristics of mental cases in pre - hospital emergency medical care in Hang- zhou, analyze the shortcomings in the treatment process and put forward strategies. Methods： Based on the emergency files of Emergency Medical Center of Hangzhou in 2011 and elaborate this paper by combining with the experiences of senior emergency physicians and emergency medical managers. Results： There are 293 mental cases of pre -hospital emergency medical care in Emergency Medical Center of Hangzhou in 2011 , which is 0.48% of the total reporting number. Male cases are slightly over women cases. The fourth quarter has the maximum, up to 34.1%. Most of them are young and middle aged. They stay on the scene for long period. Most of the cases are reported by their family members. Usually, it requires the family members and police to take compulsory measures, so emergency medical care are not taken often. Conclusion： In the men- tal cases treated by Pre - hospital Emergency Medical Care in Hangzhou, there are problems such as the disease diagnostic capability of emergen- cy staff are not qualified. Through the improvement of treatment process, protective constraint and strengthen the management of mental records, the interests of the mentally - ill patients and other emergency patients can be further ensured.
