| 摘 要： | 社区干预是降低居民慢性病发病率和死亡率的重要途径。天津市通过建立和完善慢性病监测体系、创新慢性病预防控制工作模式、培养慢性病防治队伍、建立慢性非传染病综合防治示范社区等手段，坚持以社区为基础开展慢性病控制管理，为天津以至全国的慢性病控制提供了真实可靠的数据资料和值得借鉴的宝贵经验。

| Abstract: | Community intervention is an important way to reduce the incidence and mortality of chronic diseases. Tianjin persists in cartying out community-based chronic disease control and management and provides real and reliable data and valuable experience for the control of chronic diseases throughout Tianjin as well as the country by means of the establishment and improvement of chronic disease monitoring system, the innovation of chronic disease prevention and control models, the cultivation of chronic disease prevention and treatment staffs, the establishment of model communities for comprehensive prevention and treatment of chronic non-communicable diseases. |
