摘    要:目的:探讨CD10免疫标记乳腺肌上皮细胞的能力.方法:收集乳腺良、恶性病变的石蜡包埋标本76例(纤维腺瘤、腺病、导管内乳头状瘤、良性叶状肿瘤、导管内癌、囊内乳头状癌、浸润性导管癌和浸润性小叶癌),采用免疫组化检测CD10在上述病变中的表达.结果:在41例乳腺良性病变中,CD10阳性的肌上皮细胞连续地环绕于增生的小导管的周围,敏感性可达100%(41/41).但在囊性扩张的小导管周围,CD10阳性细胞不连续,甚至不见阳性细胞.所有乳腺上皮细胞均为阴性,CD10在区别肌上皮细胞和上皮细胞时特异性为100%(41/41).导管原位癌的癌细胞巢外周的阳性细胞由完整到不完整、到完全缺失不等.在浸润性癌中,癌巢周围不见阳性细胞.除少许的癌细胞和肌成纤维细胞表达CD10外,其余的癌细胞、肌成纤维细胞、血管平滑肌细胞和上皮细胞都不表达CD10.结论:CD10标记肌上皮细胞具有较高的敏感性和特异性,可以作为肌上皮细胞的又一有效的标记物.

Abstract:Objective:To study the sensitivity and specificity of CD10 in immunohistochemical labeling breast myoepithelial cells. Methods: We collected 50 cases of breast benign and malignant paraffin-embedded samples (including fibroadenoma,adenosis,intraductal papilloma,benign cystosarcoma phyllode,intraductal carcinoma,intracystic carcinoma,invasive ductal carcinoma and invasive lobular carcinoma),and detected the expression of CD10 in above breast lesions by means of immunohistochemistry.Results:There were continuous CD10-positive myoepithelial cells around the proliferative ductules in 41 cases of benign breast lesions,the sensitivity being 100%(41/41).The specificity of CD10 in distinguishing myoepithelial cells from epithelial cells was 100%(41/41).The positive cells were discontinuous or absent in the circumference of cystic ductules.As to intraductal carcinoma,some lesions showed intact positive cells around the cancer cell nests, but others showed unintact positive or non-positive cells.No positive cells were found around the cancer cell nests of invasive carcinomas.Almost all of cancer cells,myofibroblasts,vascular smooth muscle cells and epithelial cells were positive of CD10 accept for few cancer cells and myofibroblasts.Conclusion:Being able to label myoepithelial cells with high sensitivity and specificity,CD10 may be another useful myoepithelial cell marker.
