诊断性及治疗性内镜逆行胰胆管造影的应用价值(附7238例分析)

目的：探讨经内镜逆行胰胆管造影（ＥＲＣＰ）检查的方法学及临床应用价值。方法：１９７７年１月至１９９８年６月我科对疑有胰胆疾病的７２３８例患者进行ＥＲＣＰ检查，有适应证者同时进行内镜下取石，扩张，引流等治疗，对以上资料进行全国统计，分析ＥＲＣＰ检查的临床价值及并发症。结果：７２３８例患者行ＥＲＣＰ检查共计７５７９例次，总成功率为９４．８％，胰管显影率为７５．７％，胆管显影率为８７．１％，发现胆管疾病

Practical value of endoscopic retrograde cholangiopancreatography in diagnosis and treatment of pancreaticobiliary disease

Objective: To find out the clinical value of endoscopic retrograde cholangiopancreatography (ERCP) in diagnosis and treatment of pancreaticobiliary disease and to improve the methodology of ERCP. Methods: From January, 1977 to June, 1998, with ERCP, we examined 7 238 patients suspected of pancreaticobiliary disease and for patients with therapy indication, treatments such as drainage, dilatation, lithotomy were done. All cases were reviewed retrospectively, and the clinical value and complications of ERCP were evaluated. Results: 7 238 patients examined with ERCP, totaled 7 579 times, total successful rate of ERCP was 94.8%, showing rate of pancreatic duct was 75.7%, showing rate of biliary duct was 89.1%. 3 492 cases of biliary duct disease, 570 of pancreatic disease, 821 of diverticula and 171 of fistula were found, among them 921 cases were treated endoscopically. The rate of complications of diagnostic ERCP was 1.01%, without patient death. The rate of complications of therapeutic ERCP was 1.3%, with death of 2 patients and the mortality was 0.03%. Conclusion: ERCP has important clinical value in the diagnosis and treatment of pancreaticobiliary disease.
