颈动脉狭窄的血管内支架成形术治疗

摘    要:目的:探讨血管内自膨胀支架治疗颈动脉狭窄的临床价值。方法:收治颈动脉狭窄17例,疾病症状表现为反复的短暂脑缺血发作或脑梗死。预扩张球囊通过狭窄部位扩张后回撤,自膨胀支架在导丝支撑下通过狭窄部位,回撤外鞘将支架释放,对10例患者应用不可脱卸球囊进行再扩张。结果:支架定位准确,13例患者狭窄部位恢复正常管径,3例狭窄程度减少90%以上,1例减少70%,无手术并发症发生。临床随访5-13个月(平均9.3个月),无1例再发生短暂性脑缺血发作或脑梗死。DSA随访10例患者无再狭窄(6-12个月),结论:血管内支架成形术是治疗颈动脉狭窄的安全而有效的方法。

Abstract:Objective:To summarize the experience of endovascular treatment of carotid stenosis with expandable stents. Methods:Seventeen patients with carotid stenosis experienced repeated transient ischemic attacks(TIA) or cerebral infarction were admitted to our hospital. The stenosis were pre expanded with undetachable balloon, and self expandable stents were implanted across the stenosis. A balloon catheter was used to further expand stents in 10 patients. Results:The stent was accurately implanted, total disappearance of stenosis was obtained in 13 patients,the degree of stenosis reduced by more than 90% in 3 patients and 70% in one patient. The patients recovered well and no complications related to the procedure occurred, none had TIA or infarction postoperatively. Follow up imaging in 10 patients (6 12 months) demonstrated no restenosis. Conclusion:Endovascular stenting may be a safe and valid choice for the treatment of extracranial carotid stenosis.
