冠心病心率变异特征及其影响因素

摘    要:目的 观察冠心病心率变异特征及其影响因素。方法 用24h动态电图观察40例正常健康者和66例冠同者的心率变异性。结果 冠心病患者是除PNN50和rMMSD外均明显低于对照组;心肌梗死组SDNN低于心绞痛组;变异指标SDNN,SDANN,SDNNindex,TP,LF和HF随现变加重而进行性和;前壁心梗者rMSSD,VLF,LF和HF低于下壁心梗者。结论 冠心病心率变异减少与冠脉病程度、心肌有无梗死

Abstract:Objective To investigate the features of heart rate variability(HRV) in coronary heart disease.Methods The time domain and frequency domain of HRV were studied from 24-hour Holter recordings in 40 controls(Group N) without "heart disease" and 66 patients with coronary heart disease (Group CHD) including 43 patients with angina pectoris (Subgroup ANG) referred for coronary angiography and 23 patients with myocardial infarction (Subgroup MI).The subgroup ANG had 25 patients with one vessel disease (Subgroup SCHD) and 18 patients with multi vessel disease (Subgroup MCHD).The Subgroup MI had 18 patients with anterior wall myocardial infarction (subgroup AMI) and 5 patients with inferior wall myocardial infarction (subgroup IMI).Results HRV was lower in Group CHD than Group N except rMSSD and pNN 50 (all P<0.01).SDNN decreased in Subgroup MI compared with Subgroup ANG(P<0.01).SDNN,SDANN,SDNN index ,TP,HF(all P<0.01) and LF (P<0.05) were reduced in Subgroup MCHD compared with subgroup SCHD.Subgroup AMI had lower HRV in VLF,LF,HF (all P<0.05) and rMSSD(P<0.01) than Subgroup IMI.Conclusion The HRV were related to the injury degree of coronary artery,myocardial infarction and its locative site.
