第三骶骨置钉的解剖学研究及临床应用

摘    要:目的:通过对第三骶骨的解剖学测量,探讨第三骶骨螺钉固定的安全性。方法:在32具骶骨标本上做标记,设定进钉点分别为第2骶后孔最下缘与第3骶后孔最上缘连线的中点(O点)和以O点作水平线交与骶外侧嵴的点(O’点)。骶正中嵴距进钉点的距离分别为X和X’。S3内侧置钉钉道长度为D1和D1’;S3垂直置钉钉道长度为D2和D2’;S3外侧置钉钉道长度为D3和D3’;S3横截面内侧置钉最大偏移角度为a和a’;S3横截面外侧置钉最大偏移角度为b和b’。结果:O点为进钉点,X,D1,D2,D3,a,b值分别为(19.71±1.87)mm,(20.12±2.54)mm,(15.66±2.22)mm,(28.64±5.19)mm,(33.06±9.29)°和(47.00±4.74)°;O’点为进钉点,X’,D1’,D2’,D3’,a’,b’值分别为(27.56±2.20)mm,(30.62±3.15)mm,(17.18±3.51)mm,(24.66±5.13)mm,(52.00±7.55)°和(39.00±5.50)°。结论:了解第三骶骨的解剖结构和精确的螺钉放置可以保证第三骶骨螺钉固定成功。

Abstract:Objective:To explore the security of the third sacral screw fixation through the anatomic measurement of the third sacral vertebra.Methods:The structure and contour of the third sacral vertebra were observed in 32 adult sacrum specimens.The designed entrying points were O and O’:the O point was at the midpoint betweeen the most inferior margin of the S2 foramina and the most supeior margin of the S3 and the O’point was a crosspoint which was a line from the O point horizontally toward the lateral sacral crest and intersected at the O’point.The distances from the middle sacral crest to the enterying points O and O’were X and X’.The lengths of the third sacral screw tunnel of the left and right sides passing through the median zone of the S3,the lengths of the verticcal trajectory,and the length of the third sacral tunnel of both sides through the lateral zone of the S3 on the sagittal and coronary planes were D1/D1’,D2/D2a’nd D3/D3’,respectively.The deviation angles of the third sacral screw passing through the medial zone of the vertebra and the deviation angles of the third sacral screw passing through the lateral zone of the both sides vertebra on the sagittal and coronary planes were a/a’and b/b’,respectively.Results:The values of X,D1,D2,D3,a and b were(19.71±1.87) mm,(20.12±2.54) mm,(15.66±2.22) mm(,28.64±5.19) mm,(33.06 ±9.29)°and(47.00±4.74)°,respectively.The values of X’,D1’,D2’,D3’,a’and b’were(27.56 ±2.20) mm,(30.62±3.15) mm,(17.18±3.51) mm,(24.66±5.13) mm,(52.00±7.55)°and(39.00± 5.50)°,respectively.Conclusion:To understand the anantomic structure and the precise installed location of the screw can ensure that the third sacral screw can successfully fix the fracture.
