心血池闪烁扫描术诊断原发性心肌病的临床价值(附50例分析)

本文报告了确诊的50例原发性心肌病病人的心血池扫描结果。肥厚型心肌病的心血池扫描图象表现为3种类型,即狭窄型15例,稀疏型14例,正常型2例。狭窄型为梗阻型肥厚性心肌病的典型表现;稀疏型则为非梗阻型肥厚性心肌病的表现;而正常型视为假阴性。故心血池扫描诊断肥厚性心肌病的灵敏度为96. 0%。19例扩张型心肌病的心血池扫描图象特点为心血池影扩大,呈球形或椭圆形。但缺乏诊断的特异性。当怀疑有扩张型心肌病而又不能确诊时,心血池扫描对诊断有较大价值。此技术不仅对原发性心肌病的诊断,而且对梗阻型与非梗阻型肥厚性心肌病以及肥厚性与扩张性心肌病的鉴别诊断,也提供客观依据。

DIAGNOSTIC VALUE OF CARDIAC BLOOD-POOL STATIC SCINTIGRAPHY IN PATIENT WITH IDIOPATHIC MYOCARDIAL DISEASE (WITH ANALYSIS OF 50 CASES)

The results of cardiac blood-pool static scintigraphy(CBPS)in 50 patients with idiopathic myocardial disease(IMD)clini-cally diagnosed were reported.Scintigram of 31 patients with hyper-trophic cardiomyopathy(HCM)showed three patterns(15 stenosis,14reduction and 2 normal).The stenosis pattern was the typical appearanceof hypertrophic obstructive cardiomyopathy(HOCM);The rare patternwas the appearnce of hypertrophic non-obstructive cardiomyopathy(HNCM),and the normal pattern may be negative falsely.The diagno-stic sensitivity of CBPS for HCM thus was 96. 0%.CBPS showed roundor elliptic shape and dilated heart in 19 patients with congestive car-diomyopathy(CCM).It was more useful for evaluation of CCM butabsence of diagnostic specificity.While diagnosis was suspected but wasnot determined,it had higher values.CBPS may provided objective evidence not only for diagnosis ofIMD but also for differential diagnosis between HOCM and HNCM aswell as HCM and CCM.
